  • Policy lapsed / expired.
  • Policy turns to extended term insurance.
  • Admission before risk commencement date.
  • Covered Event falls within 30-days waiting period or 120-days specified illness.
  • Diagnosis falls under Policy Exclusion, eg. pregnancy related illnesses, congenital & etc.
  • Pre-existing Condition is suspected.
  • Non-disclosure of material fact.
  • Non-panel hospital.
  • Overseas treatment.

Yes, this is at hospital's discretion and the amount varies from each hospital. This amount is used to offset any non-covered items during discharge and any balance is refunded back to member.

Yes, the member is required to self-pay and submit for reimbursement claims to Etiqa Medical Claim Department. Claim will be assessed in accordance to terms and conditions of the policy contract.

  • Claim form
  • Medical Form - Statement of Medical Examiner
  • Investigation report (if any)
  • Original bills
  • Original receipts (this is very important as claims can be repudiated in accordance to policy contract)

You can request for Guarantee Letter by providing your identity card number or policy number to the hospital staff. The hospital staff will contact Etiqa Healthcare to verify the policy status and eligibility.
